At your first visit you will be asked to complete a personal and medical information form. This information is confidential and will be provided to others only with your specific permission.
The information we request will help us in the diagnosis and treatment of your problem and will help you as well. Your past medical history may play an important part in the treatment plan which your doctor will formulate to best suit your individual needs.
Because some of the treatment provided by oral, surgeons is paid under medical insurance, we ask you to give us your medical as well as your dental cards and/or required forms. Before surgery you should read your benefits booklet or consult with your insurance representative to be clear about your coverage.
