At your first visit you will be asked to complete a personal and medical information form. This information is confidential and will be provided to others only with your specific permission.
The information we request will help us in the diagnosis and treatment of your problem and will help you as well. Your past medical history may play an important part in the treatment plan which your doctor will formulate to best suit your individual needs.
Because some of the treatment provided by oral, surgeons is paid under medical insurance, we ask you to give us your medical as well as your dental cards and/or required forms. Before surgery you should read your benefits booklet or consult with your insurance representative to be clear about your coverage.
Knowing all the details of your current insurance coverage may help us to submit your claim accurately and insure maximum reimbursement under your plan(s). Please refer to our practice Insurance Brochure for more details about our policies regarding insurance.
Feel free to ask us anything regarding your diagnosis or treatment plan. Your doctor will clearly and frankly respond to all your questions and give you all the information you require to make good, informed decisions about your care.
At the time of your first visit we ask that you bring the following:
- A copy of each of the insurance cards and/or forms required by your insurance plan(s). Be sure to bring both dental and medical insurance information, primary and secondary.
- If you are an HMO patient, your plan generally requires that you obtain a referral from your primary doctor before you seek specialty care. You must have the referral with you, in order to be treated.
- If you have a medical condition and are taking medications, bring a list of all those medications as well as a list of any drug allergies.
- Any X-rays or test results which pertain to your condition.
